Service Description

Prior to appointment, a health history form will be provided. Completion of this form prior to the scheduled appointment helps to pinpoint what areas the client would like to work on. This includes physical, emotional, spiritual, environmental, etc. This background information helps to assess and pinpoint the root cause of an injury (physical or emotional).
